Ethical approval

All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional and/or national research committee, the 1964 Helsinki Declaration and its later amendments, and comparable ethical standards. The CAIN trial was approved by the Institutional Review Board (IRB); formal IRB approval was waived for the 1-year data collection for this case report.
